The increased risk of obesity among families with school-aged children of Hispanic
descent related to knowledge deficit of nutrition, as demonstrated by the increased obesity rates
of the Hispanic population of Nacogdoches County.

Obesity is an ongoing health concern in Nacogdoches County that is noted by the
increased number of school-aged that are considered overweight. There is a health inequity
observed in the County that shows that the children of Hispanic descent have a higher occurrence
of obesity due to cultural, economic, environmental, and knowledge deficit. The Hispanic culture
has a high in calories diet, which is a significant reason obesity rates are high in the community.
With only about 10% of the Hispanic population graduating college and around 50% graduating
high school, many Hispanic families are faced with the challenge of focusing on the economic
value of their meals instead of the nutritional value. There are many fast-food restaurants and
stores that offer foods that are high in calories and fats in the surrounding area that are easily

accessible. There is also a knowledge deficit in nutrition due to conflicting information from
various resources and being afraid to ask questions regarding their nutritional needs.

As I was completing my field experience and site assessments, I explored the resources
that my community had to support the promotion of healthier food choices and the prevention of
obesity in my target group. First, there is a supplemental governmental program for women,
infants, and children know as the WIC program. This program supports healthier food options
for pregnant women, infants, and children younger than five years old. The WIC clinical director
explained the process of becoming a clientele of the WIC program, the food packages that are
currently available, and the educational and nutritional education classes provided with their
services. In addition, the local Parks and Recreation Department offers a broad range of physical
activity classes, recreational sports, and occasional cooking classes. Finally, the Healthy
Nacogdoches Coalition promotes school gardening activities that encourage school-aged
children to grow vegetables so that they might be vested in eating healthier. In addition, other
resources such as a local farmers’ market, health food stores, and even some roadside vendors
provided healthy food choices to prevent obesity in the community. The primary goal is to
encourage health and wellbeing by educating the community on healthy eating habits to prevent
diseases caused by weight gain.

Obesity has many factors that contribute to it. The Hispanic community’s diet consists of
many foods with a high caloric intake with flour, rice, or corn served with most meals. Many are
unable to afford healthier meal choices due to the higher costs of fresh fruits and vegetables. One
of the most significant factors of obesity in the Hispanic community in Nacogdoches county is
knowledge deficit. “The mothers and wives that cook most of the meals do not understand basic
nutrition. Some do not even know what a protein is.” is what Susan McDonald, Ph.D., RN,
CPNP, said to me when we discussed her research article titled The Lived Experiences of
Mexican-Heritage Mothers Caring for Overweight Preschool Children. She also went on to say,”
nutritionists and providers are giving conflicting information, and it is usually the providers that
are providing the wrong information to the patients. However, the mothers are not questioning
the differences.” If the community does not know about nutrition, their diets are not balanced,
causing obesity and other health-related problems later in life.

Eating patterns in the United States are far below Dietary Guidelines recommendations.
Diet-related chronic diseases, such as obesity, pose a significant public health problem for
Americans (United States Department of Agriculture [USDA], 2020). Nutritional education is
essential when dealing with lifestyle changes that affect your health. It takes 10 to 15 hours of an
educational intervention to improve nutrition knowledge, 40 to 50 hours for changes in eating

behaviors, and 26 hours of intervention for weight loss in children and adolescents over six years
old (Franciscato et al., 2019). The earlier you can get children invested in eating healthy, the
greater the chances are that they will continue to eat healthily. The local school district has
partnered with The Healthy Nacogdoches Coalition to start a gardening program at the
elementary school level to get the students involved with making food choices in conjunction
with the nutrition classes that the students receive in school.

Nacogdoches County in the State of Texas has a population of 65,558 people. The
average household income is $43,558, with 24.4% of the population below the poverty level.
Additionally, 18.9% of the residents are uninsured. Several people in the community are
concerned about obesity in the County due to 35.1% of adults being obese. In 2014 the rate was
29.8%, an increase of 5.3%. This data supports the community’s concerns concerning obesity
and related diseases that occur because of being obese. Many resources in the community
support issues regarding food insecurity; however, most residents have little access to free or
reduced-cost nutritional classes. Most (mothers) want to do what is best for their children, they
are aware of the hurdles, and they expect health care professionals to help make their children
healthier than previous generations (McDonald, 2015). Therefore, education to promote healthy
eating habits and lifestyle changes is needed to decrease obesity.

Social media is used throughout the world to connect people and share information. My
social media campaign would be to promote nutritional education programs to members of the
Hispanic community in Nacogdoches County to prevent obesity. The campaign’s effectiveness
can be assessed by looking for a decrease in BMI within the Hispanic community within 120
days. My social media campaign would consist of live sessions and daily posts that would
educate the Hispanic community about nutrition.

My first social media intervention would be to have live online sessions for the Hispanic
community. To do this, I would need to explain my mission to the priest. They are the most
influential and trusted people in their community. Next, I would get them to make social media
videos that would promote the nutritional educational classes offered. The live online sessions
would include basic nutritional information. My subsequent intervention would use my social
media page to promote nutrition through a cooking demonstration that uses culturally appropriate
meals. By using social media, I hope to reach the greatest amount of the Hispanic population,
help them become competent in nutrition, and reduce the obesity rates in their population.

I would begin my social media platform using Facebook. The platform would offer
nutritional education content that would be appropriate for different ages. Using Facebook would
allow me to have live and recorded educational classes as well as cooking videos. In addition,
followers could share the information they learned with others in their community.

Social media has benefits that include viral sharing of information, social engagement,
and the ability to reach new people quickly. Social media not only allows information to be
shared but also allows feedback on the information. The sharing of feedback would enable
followers to gain a trusting relationship, encouraging followers to make healthy nutritional
choices and lifestyle choices that would help prevent and control obesity rates in their
communities.

When using social media to convey nutritional information, it is vital to understand my
target population of school-aged children and young adults in the Hispanic community of
Nacogdoches County. The objective of targeting school-aged children is to get them invested in
eating healthy while they are young. The more that they can learn as children can grow with
them throughou9t their lives. Targeting young adults helps by showing them how to make the
lifestyle changes needed and encouraging them to continue the healthy behaviors learned
throughout their nutritional classes. The Hispanic population wants a healthier life for their

children. They need the tool to help them. Using social media would allow the spread of
education that would be considered functional at this time.

People use social media to be part of a community. Therefore, using social media
to connect to people dealing with the same medical conditions plays a vital role in their
lives. People with chronic illnesses or living with specific needs, like obesity, often feel like they
don’t belong due to their conditions. That is the power of social media. It can connect people, so
they do not feel alone. Facebook groups can be developed to help people feel free to share
information, meet other people, and support each other (Popa, 2020).

Key stakeholders would include the local Department of Health and the leaders in the
Hispanic community. The health professional at the local DOH could provide their professional
input and would be responsible that all information on the Facebook page is in accordance with
governmental guidelines. In addition, the local Hispanic community leaders can offer appropriate
cultural aspects that deal with the ethnic diets. Also, both stakeholders could help get the

information about the educational classes out to the community and coordinate gatherings or
activities.

The prominent partnerships would include the local Department of Health, the local
school districts, and community leaders. The local DOH could help with educational materials
already translated into Spanish. The local school systems could provide educational time during
school hours that incorporate everything from growing nutritious vegetables to cooking and
having fun learning materials. Finally, community leaders could help spread the word about the
campaign because they are the ones that the Hispanic community trusts the most.

The social media campaign could be running within a couple of months. First of all, I
would need to gather statistical data to present to all stakeholders. During this time, I could
develop the Facebook page and start sharing links to myplate.gov. My next step would be to

invite all my stakeholders to a group meeting to pitch my ideas and inform them of the benefits
of this campaign for the Hispanic community. This time would allow them to give any additional
input on the contents of the page. I would then use the community leaders to spread the word of
the new Facebook page and get them to encourage their communities to engage with it. Finally, I
would need to find a native “face” to present the educational material correctly by somebody that
the community could trust.

We can evaluate the effectiveness of the educational program by comparing the BMI
rates over time. We could collect our information by using a survey on our social media
platform. The data collected could be turned into a percentage. A healthy BMI is between 18.5
and 24.9 kg/m². We would be hoping to see a steady decline over this marker over the next six
months. We could also have surveys asking basic nutritional knowledge to determine if our
educational materials are effective. Using these tools, we could decide if our followers have
made lifestyle changes to control obesity and its related diseases.

The initial funds needed to start my social media campaign would come from grants and
donations that I could find in my community. My team would initially consist of voluntary
members of the community concerned about the obesity rates of the Hispanic population in my
County. In addition, educational materials can be obtained from various governmental agencies
and local nutritionists, and local hospitals. Of course, the Social media page does not require any
necessary funds to create, and by using local community leaders to spread the word, there should
not be the need to acquire advertisement. Therefore, only time and travel expenses would
contribute to the overall cost of the educational program at the beginning.

Social media has become a preferred method of spreading information to a diverse
amount of people throughout the world. For example, one can send a message via Facebook
faster than sending a letter in the “regular” mail. With so many Americans on social media, it has
been proven an effective platform to educate and discuss current health concerns. Evidencebased practices can be applied to the data obtained from the feedback provided and can be used
to adjust the materials needed to control obesity. Thus, ensuring that our educational materials

could reach a more significant number of people. Social media applies to future nurses by
allowing them to start conversations online with experts and the people that wish to improve
their lives. Future nursing practices must transform into whatever “new” technology is created as
it is constantly evolving.

B: Nursing Diagnosis Statement
My community health nursing diagnostic statement is increased risk of hospitalization
among obese Wilson County residents related to COVID-19 as demonstrated by higher mortality.
B1: Health Disparity Among Wilson County, TN
There are approximately over 144,000 residents in Wilson County of Tennessee. Of those
habitants, 34% are labeled as obese (Tennessee, 2021). This is slightly higher than the state
average of 33% per capita. Obesity is trending higher in recent years for Wilson County. Obesity
is described as an excess aggregation of body fat. This can compromise health significantly.
Obese persons are prone to decreased airways due to limited chest expansion, which
compromises ventilation. Therefore, oxygen use decreases and causes weakening of the
respiratory system (Maronezzi da Silva et al., 2021). According to the CDC, obesity may triple
the risk of hospitalization due to COVID-19 infection (Obesity, Race/Ethnicity, and COVID-19,
2021).
B1:A- Primary Community and Prevention Resources
Prevention of COVID-19 is relatively easy for residents of Wilson County. Wearing a
mask, social distancing, washing/sanitizing hands often, and getting a COVID-19 vaccine are all
primary ways to prevent COVID-19 infection (How to Protect Yourself and Others, 2021). Upon
several site assessments, there are many stores in Wilson County that are still selling masks, hand
sanitizer, and hand soap readily. Most stores still have hand sanitizer available for customer use
and practice social distancing by signage on the floors. There is fabric for sell at several stores
for those that prefer to hand-make their own masks. COVID-19 vaccines are available at the
Wilson County Health Department clinic, numerous pharmacies, and many healthcare clinics.

Prevention of obesity in Wilson County is more challenging. There is a considerable
amount of fitness gyms available for use, however, there are none available for free use. There
are several parks accessible, some of which have nature walking trails. The crime rate is
relatively low in Wilson County, so residents may feel comfortable walking in their
neighborhoods to get exercise. Fresh fruits, vegetables, and meats are easily found at all grocery
stores. There are quite a few discounted grocers as well. As previously stated, obesity in Wilson
County is trending upward in recent years. The COVID-19 pandemic most likely caused an
increase due to “stay-at-home” mandates and multiple businesses being required to close.
Although now that businesses are re-opened, residents may feel hesitant to join a gym or attend a
recreational activity because of risk of COVID-19 infection.
B1:B-Underlying Causes
The underlying causes of obesity are from behaviors (dietary and physical activity),
community environment, genetics, diseases, and drugs (Adult Obesity Causes and Consequences,
2021). If one is in the habit of not eating healthy or getting plenty of exercise, they are at risk for
obesity. The safety of the community is critical to whether an individual chooses to get physical
activity in their neighborhood or if they remain inactive. Cost of food and availability of healthy
food choices are also important factors to prevent obesity. Genetics and family history can predispose one to obesity. Certain diseases, such as polycystic ovarian syndrome, make fighting
obesity very difficult. Some medications, such as steroids, cause weight gain. The effects of
obesity can be detrimental. The following are common consequences of obesity: hypertension,
dyslipidemia, diabetes, coronary heart disease, stroke, osteoarthritis, gall bladder disease, sleep
apnea and breathing difficulties, various cancers, low quality of life, mental illness (depression,
etc.), body pain, and mortality (Adult Obesity Causes and Consequences, 2021).

B:2-Evidenced Based Practice
According to the CDC, obesity is linked to impaired immune function and decreased lung
capacity, thus, increasing risk of hospitalization (Obesity, Race/Ethnicity, and COVID-19, 2021).
The Western Journal of Emergency Medicine provides evidenced-based data in their research
report for young adults diagnosed with COVID-19. They report that obesity is identified as a risk
factor for the severity of viral illnesses (Steinburg et al., 2020). Results from their study of 210
individuals diagnosed with COVID-19 include: 18 (9%) died during hospitalization, 35 (17%)
required mechanical ventilation, and 94 (45%) were admitted to the hospital. Of note, the
research found that the patients who died during hospitalization had a BMI higher than any of the
other participants in the study. The study concluded that obesity is an independent risk factor for
poor outcomes in young patients with COVID-19 (Steinburg et al., 2020).
B:2-Data from Wilson County, TN
Since March 8, 2020, there has been 19,308 cases of COVID-19 reported and 243 deaths
in Wilson County. In the last seven days, there has been 36 reported cases and no deaths (Wilson
County, TN Coronavirus Cases and Deaths, 2021). The state of Tennessee has had 865,085 cases
of COVID-19 since March 8, 2020, and 12,498 deaths as well. As previously mentioned, the rate
of obesity in Wilson County is 34% and has been increasing in recent years. The average rate of
obesity for the state of Tennessee is 33% (Tennessee, 2021).

My social media campaign objective would be to decrease obesity in Wilson County, TN.
Because obesity has been trending upward, my goal would be to have obesity trend downward. I
would like to have the current rate of obesity to decrease from 34% to 30% within two years.
C:2-Social Marketing Interventions
My social marketing interventions would be to use Facebook and YouTube as my social
media platforms. These two websites are extremely popular and accessible to all ages. It is easy
to post a link to Facebook that will take the user to a YouTube video. It is also easy to share a
link from YouTube to Facebook. All age groups tend to use Facebook to keep in touch with
family and friends, therefore, the target audience would be broad in age ranges. YouTube has a
search feature that allows the user to search for certain videos that pertain to the subject they are
interested in. Many people of all ages use YouTube as a way to learn new things, catch up on the
latest news, or as a form of entertainment. I could construct a Facebook page for Wilson County,
TN and use this page for promotion of weight loss techniques and activities that still allow for
CDC guidelines of the prevention of spread of COVID-19. I could pay for ads to be shown in the
user’s newsfeed on Facebook that is promoting weight loss strategies geared toward Wilson
County residents. This Facebook page would provide links to my YouTube channel where I
would have informational videos about weight loss and promotion of activities for Wilson
County.
C:3-Social Media Platform
I would choose Facebook as a primary way of promoting to the target audience of Wilson
County, TN. I would create a Facebook page called, “The Wilson Wellness Project”. I would pay
for ads to be displayed in Wilson County resident’s newsfeed. The ads would be promotion of the
Facebook page and for the user to “like” the page. After the user “likes” the page and begins to

follow the content, they would be exposed to an abundance of information about how to lose
weight and eat healthy. I would organize activities in Wilson County, such as a 5K Walk/Run,
and promote those activities on the page. There would also be creative ways to promote exercise
while abiding by CDC guidelines to prevent the spread of COVID-19. My Facebook page would
also have links on the page that takes the user to my YouTube channel for more in-depth
information about obesity and weight loss and prevention of the spread of COVID-19. Facebook
also has the “stories” option that users could watch for more information after following my
page. I would keep the “stories” feature active and current daily.
C:3A-Benefits of Selected Social Media Platform
The benefits of choosing Facebook as my primary social media platform are that many
people use Facebook of all ages. A lot of people use Facebook for multiple reasons. Often, users
will hear the latest breaking news from Facebook before a news media network. The algorithm
for Facebook is to have an ad placed in the newsfeed of the user for every 2-3 posts. Often these
ads are repeated frequently. Facebook can view the user’s Google searches and then promote ads
on Facebook that have products or services the user searched for on Google. Therefore,
Facebook could pinpoint users that reside in Wilson County from their Google searches or if they
list their current location in their profile. I could pay to have Facebook ads in the newsfeed of my
targeted audience of Wilson County. These ads could be repeated numerous times so that users
will notice them and go to my Facebook page. “The Wilson Wellness Project” page would have a
wealth of information about obesity, weight loss, exercise, and COVID-19 prevention guidelines.
C:4-Benefit to Target Population

The benefit of my social media campaign for Wilson County is for the residents to learn
information and apply it to their lives. The goal is for Wilson County to reduce the obesity rate.
The use of Facebook is free for all users with a smart phone or computer. Therefore, the wealth
of information being promoted on “The Wilson Wellness Project” page would be free to all users.
Facebook is used by a wide variety of ages and the page could be viewed easily by users of all
ages. Users would also be able to “share” information from the page to friends and family and
help promote the message of weight reduction and COVID-19 prevention.
D: Best Practices for Social Media
The best practices for implementing social media for health marketing are to use the
platforms that are most widely used and easiest to use. By making the Facebook page and
YouTube channel personalized to Wilson County, residents may feel more of a connection and
spark more interest in the campaign. The information about weight loss and COVID-19
prevention should be scientifically based facts but promoted in an engaging and pleasant way.
The information should be eye-catching and personable. The goal of obesity reduction in two
years would be made known to the public and made as a challenge to residents to achieve and/or
surpass this goal.
E:1-Stakeholder Roles and Responsibilities
The key stakeholder I would choose would be Vanderbilt University Medical Center. This
hospital is also a prestigious college that has multiple medical programs for careers. Vanderbilt is
a widely known and trusted college and hospital system and happens to have a hospital and
multiple medical clinics located in Wilson County, TN. Vanderbilt conducts many studies and
trials for various medications and methods of medical practice. Vanderbilt is also a wealth of
knowledge and information and could serve as a trustworthy resource for the campaign and also

support it financially. It is my belief that Wilson County residents would find my social media
campaign respectable and reliable with Vanderbilt Medical Center associated with it.
E:2-Public and Private Partnerships
A potential public partnership would be Facebook and YouTube social media platforms.
Both websites would be able to collect data to use for marketing, algorithms, and promotion of
ads. Potential private partnerships would be at a local level. I would consult with local grocers to
see which business would be interested in partnering. Promotion of their business would include
a discount on healthy foods, such as fruits, vegetables, and meats. The discount would attract
more customers to their business, and they will, in turn, make a higher profit. I could also
network with local gyms and ask for a discounted membership for at least the first year. This
could benefit their business as well by bringing in more members.
E:3-Timeline for Implementation
My goal for implementation of my social media campaign would be one month. It would
take some time to form the Facebook page and YouTube channel. I would need a team of
volunteers to assist with social media moderating and coordination of activities at the county
level. I would need time for filming videos and editing the videos. I would also need to reach out
to Vanderbilt Medical Center as my key stakeholder and encourage their participation. Next,
would be spending time networking with local businesses in search of partnerships. The goal
would be to have this all completed and active in one month.
E:4-Evaluating the Effectiveness
In order to evaluate the effectiveness of my social media campaign, I would first look at
the data from the Facebook page and YouTube channel. Both websites provide data about the
level of reach of users and the engagement of users. I would check the views of the YouTube

videos and see how many users have subscribed. I would also check how many users are
following the Facebook page. This gives clear data of how well the social media outreach is
going. In order to evaluate the long-term effectiveness, I would rely upon the Wilson County
Health Department for data analysis of obesity rates within the two years following the launch of
the social media campaign. Ultimately, the data from the health department would prove how
effective the social media campaign is doing by helping Wilson County to lose weight and
reduce obesity.
E:5-Cost of Campaign
The cost of implementing this social media campaign would be relatively low. The
highest cost would be for paid ads to repeat on Facebook. Because of the change of algorithms
on Facebook, pages now need to pay to get followers to see their posts or have ads in potential
follower’s newsfeed (Cooper, 2021). If a user is active and engaging with a page, they will see
the page’s posts frequently. But if the follower is not active, they will only see posts rarely.
Therefore, that explains why Facebook requires pages to pay a fee for promotion of their page
and for ads to appear in non-follower’s newsfeed (Cooper, 2021). YouTube seems to have the
same algorithm with subscribers. If subscribers are not engaging with the channel, they are not
likely to see new videos in their feed. Therefore, this is the area I foresee the highest cost for the
campaign.
The next highest cost does not involve monetary value. The cost would be the volunteer
time it would take to coordinate county activities and manage social media platforms. It would
also be a lengthy amount of time to film and edit YouTube videos. It is my hope that all
volunteers would gladly sacrifice their personal time within reason to devote to this social media

F: Reflection on Social Media Marketing
The community health nurse can benefit from social media marketing by identifying a
target audience he/she wants to reach and promoting up-to-date health information to that
audience. Because most people use some form of social media platforms currently, it is easy for
the nurse to share critical information to a numerous amount of people. Most social media
platforms can share posts or videos to other social media platforms. Therefore, the reach of the
target audience broadens even more because of the “shareability”. This makes social media a
substantial method of promoting life-saving information to a wide range of patients.
F:1-Reflection on Future Nursing Practice
As a dialysis nurse, I could use social media to promote ways to prevent kidney failure to
the public. Many people are not aware of the most common causes of kidney failure, diabetes
and hypertension. There are a numerous amount of patients that do not know they have
hypertension or diabetes. Through the use of social media, I could provide education about
kidney failure, how to prevent it, and how to recognize symptoms of uremia. I would be sure to
make my content fun and appealing to encourage user engagement and shares of content. My
goal would be to provide successful mass communication of valuable health information.
