Who is ready to play detective? You are going to give clues and be detectives with diagnostic laboratory values. For your initial post you will post common assessment data that you may ascertain in the exam room and the abnormal results of a diagnostic exam(s) that relate to a specific disease but do not give away the specific disease. Your fellow students will then be the detectives to determine the diagnosis. After the diagnosis is determined (remember to go back and check if your classmates choose the correct diagnosis) your fellow students will discuss if the diagnostic exams were cost effective in diagnosing the disease and if any further exams need to be order.

Knowing where to find the information that you need is a very important tool to use when in the primary care setting. Different resources can help you effectively care for the client that presents with non-specific situations. These tools can help you determine additional assessment data that may be needed, develop a list of differential diagnoses, list of diagnostic exams that may be needed and also help with your plan of care. Perform an internet search on the different resources that are available to use when you are working if your practice area. Manage a plan of care for two of the possible differential diagnoses including determining what the results of the diagnostic exams would be.

In this discussion you will create an age appropriate, evidence based plan of care for a client that might present to a primary care setting with a chronic disease or diseases. Make sure to include in the case study subjective, objective, and assessment data. You will post this information to the Discussion Board. Please provide a list of differential diagnoses, diagnostic exams and create an evidenced based management plan for the client. Be sure to provide a medication list and appropriate dosages. Remember when creating a plan that this patient will have more than one chronic disease and you should create a plan to involve all aspects of care. Comment on peers plan of care in terms of cost effectiveness and evidenced based.

I-P: Hispanic male who presents with Diabetes Type I and a HGB A1C of 9. This patient also has Chronic pain related to Peripheral Neuropathy and Unstable angina related to coronary Artery Disease.
Q-Z: African American male with a 10-year history of Chronic Kidney disease, Diabetes Type II, and uncontrolled Hypertension with BP’s ranging from 170/90–200/110.
