Healthcare costs continue to skyrocket as the government and healthcare facilities
scramble to find ways to reduce costs. With the implementation of the Affordable
Care Act, emphasis is supposed to be placed on illness prevention rather than
disease management. As an RN in a behavioral health facility, I see recidivism due
to nonadherence as a major issue and drain on healthcare costs. Green (2017)
defines recidivism as “relapse into a previous condition, or mood or behavior
(return visits to a crisis prevention center, or a long-term treatment facility
for an individual with mental illness).” An overwhelming amount of patients
seen on the inpatient unit on a daily basis have had multiple hospitalizations within
a one year time frame. For example, the Agency for Healthcare Research and Quality
(n.d.) states that 22% of hospitalized patients with schizophrenia will be readmitted
within 30 days. Many patients cite poor access to treatment as a major cause to
hospital readmission. This impacts the hospital’s bottom line overall because
insurance reimbursements are decreased with multiple readmissions and impacts
the caregivers as we see the same patients on a regular basis get better only to come
back sometimes days later in worse shape than they had been prior to the last
hospitalization. The National Alliance on Mental Illness (n.d.) projected that in
2019, serious mental illness caused $193.2 billion in lost earnings each year
throughout the U.S. economy.
My current facility has a large reach in the community and provides several outpatient
options. Current COVID precautions have caused an increased strain on resources for
patients as many facilities are closed or contactless. As a result, my facility has
increased their telehealth capabilities to allow for greater access. Also, recently many
patients have been getting their prescriptions filled via the hospital pharmacy prior to
discharge to encourage medication adherence after discharge and decrease any
chances of missed doses. In addition, patients are discharged with an appointment for
aftercare already confirmed and patients discharged to the facilities intensive outpatient
program are transported directly to their intake appointment for aftercare.

A national healthcare issue that I decided to analyze is one of the most talked-about and, I have
experienced since I began my nursing career is the nursing shortage. I have work different units and
hospitals and being short-staffed is not fun. On my med surge floor the nurse to patient ratio is 1: 4 but
due to not having enough nurses, a nurse can have up to 6 patients, oftentimes not stable patients, given
the nature of the current situation we have with COVID, things are only getting worse, because nurses are
leafing their regular job to go into travel nursing due to the pay increase, and this only makes the job more
difficult and stressful, and patients are the ones who are left to suffer. With the nursing shortage, it is very
difficult for patients to receive excellent care due to not having enough nursing bodies on staff.
According to the Bureau of Labor Statistics Employment Projections 2019-2029, Registered
Nursing (RN) is listed among the top occupations in terms of job growth through 2029. The RN
workforce is expected to grow from 3 million in 2019 to 3.3 million in 2029, an increase of
221,900 or 7%. The Bureau also projects 175,900 openings for RNs each year through 2029 when
nurse retirements and workforce exits are factored into the number of nurses needed in the U.S.
With the nursing shortage, as a nurse, this really makes it very hard to have a personal life, because
we are asked and forced to work extra hours. Often a nursing assignment becomes unfair due to
high patient acuity. I see so many of my colleagues get burnt out due to the workload. My
healthcare system has implemented new staffing to include bringing in nurse travelers to help with
the workload. But with the travelers can also be challenging because, the travelers want to work
specific days and shift and that often do not cover the needs of my unit, with the nursing shortage
which affect almost all part of my health system, when my unit has an extra person to help, that
person gets floated to another unit dues to call-outs and so on. The nursing shortage is really
becoming a national crisis and with the pandemic and new increase in pay for travelers one area, or
hospital might be well-staffed due to the pay package for its travelers, while some areas will still
have nurse staffing issues.
